I certify that I am 18 years of age or older, or am the legal guardian signing on behalf of a minor aged 10–17 who meets all other requirements below.

I certify that I weigh between 90 and 265 lbs (41–120 kg), which is the mandatory safety rating for this equipment as specified by the manufacturer. I understand that use outside this range creates a life-threatening risk of equipment failure.

I certify that I am in adequate physical condition to navigate strenuous, vertical terrain and that I am free of any medical conditions — including but not limited to heart conditions, seizure disorders, vertigo, or impaired balance — that would prevent me from safely participating in via ferrata climbing.

I understand that GearDash LLC is solely an equipment rental company and does NOT provide guide services, instruction, safety assessments, or any evaluation of my fitness or climbing ability. I am solely responsible for determining whether I am capable of safely using this equipment and navigating the route.

I understand that via ferrata climbing involves INHERENT RISKS including falls, rockfall, lightning, equipment failure, human error, and death. I voluntarily accept all such risks.

I agree to inspect all rented equipment upon pickup and will not use any gear I believe to be damaged or defective. I will return all gear to the designated Smart Locker in the same condition as received, and I accept a $150/day overdue fee and full replacement cost for lost or damaged gear.

I RELEASE, WAIVE, AND DISCHARGE GearDash LLC, its officers, members, employees, and agents from any and all liability, claims, and causes of action — including negligence — arising from my use of rented equipment or participation in via ferrata climbing at Ashley Gorge, Vernal, Utah.

I understand that by checking these boxes I am making legally binding certifications. I acknowledge that providing false information on this document constitutes fraud and that any injury resulting from a false certification is my sole legal responsibility.
